1. Before the mobility
Table A – a table with the program agreed before departure. Please enter all items that will be processed during the exchange. The number of ECTS points should be agreed with the host university.
Table B – a table with the program that is implemented at the Faculty of Pharmacy of the Jagiellonian University Medical College and which will be credited (replaced) with the study program implemented as part of the Erasmus+ mobility at the host university abroad.
2. During the Mobility
Table A – please enter each change of subjects from the Before the mobility section (Table A) – previously agreed with the faculty coordinator of the Erasmus+ program of the foreign university.
Table B – enter each change of subjects from the Before the mobility section (Table B) – previously agreed with the faculty coordinator of the Erasmus+ program of the home university. If the modifications to table B do not result from changes shown in table A, you can leave it blank or mark “no changes”.
3. After the mobility
This is the part that must be completed after completing your studies abroad.
Table C and Table D – this is a list of credits obtained, agreed with both the host and home universities.
Additional information:
Each student who intends to pursue part of their studies at a foreign university is obliged to submit an application to the Vice-Dean for Student and Teaching Affairs before departure. The application must be accompanied by a syllabus of a given subject from both the foreign and home universities. The decision to include a subject in the course of study is made by the Vice-Dean for Student and Teaching Affairs, taking into account the learning outcomes achieved. The application should be submitted to the Dean’s Office, room 4.
Settlement of the exchange stay under the Erasmus+ program
The following documents will be required to settle the exchange stay:
1. Learning Agreement for Studies part of After the Mobility
2. Report (4-5 pages) presenting an introduction to the research area, assumptions, purpose of the work and description of the results, literature. It is important that the scope of disclosure of results/results is agreed with the supervisor and does not violate the principles of confidentiality – applies to students carrying out a research project
The indicated documents should be submitted to the Dean’s Office, room 4.
